My lucky bamboo plant stems are rapidly turning yellow and soft. What on earth can I do to save them? Thank you!

Sounds like the vascular tissue is breaking down most likely caused by a plant pathogen unfortunately in many cases a pathogen can move through the plant very quickly irreversibly destroying cells. Changing the water on a weekly basis will help keep this from happening, filtered water seems to work best.
